Schedule an appointment.

 - An appointment will be scheduled for an evaluation and you will receive directions to our clinic. We ask that children arrive in comfortable play clothes in order to have full range of motion for testing and their comfort.
We will do our best to find a time that is convenient for you and your child’s schedule. After the initial evaluation we will try to find a permanent time that is consistent week to week.

- Our staff will email you a welcome email with all our paperwork for you to fill out and get back to us.  You can then email or fax it to us.  They are fillable PDF forms, so it should be fairly easy for you to fill them out and send back to us without printing them.

Included in our paperwork you will find:
Parent Questionnaire
Consent for Treatment/Medical Release
Privacy Rights (HIPAA)

Media Release

Medical Information Release

Cancellation Policy
Sensory Processing Disorder Checklist (if applicable)
Please read all forms thoroughly so that you are informed about the agreements you are signing, and ask any questions to better help us serve you and your family.

If you would like a copy of our forms, please fill in the information below and we will get them to you as soon as possible.

3rd Step

What to Expect

Obtain a referral or script from your child's physician.

-Either our staff or you can contact your doctor to facilitate getting a referral and/or script. Typically it needs to say "OT (or PT or ST) to evaluate and treat"

4th Step

Therapy Initial Evaluation

Allow at least an hour, up to an hour and a half. Please arrive on time and make yourself comfortable in our waiting room. Your evaluating therapist will meet you and your child there to provide a tour of our space to help familiarize your child to our environment. The therapist will use standardized and non-standardized tests, observation and parent/physician information to get an overall snapshot of your child's strengths and weaknesses. Parents are strongly encouraged to contribute to the goal-setting process.

Once the evaluation is completed the therapist will discuss preliminary findings and offer some recommendations.  Once the insurance authorization is obtained (if applicable) and the doctor has reviewed and authorized the evaluation, we will schedule your child for a weekly appointment for therapy. Every attempt will be made to find a mutually agreeable time for the appointments. Generally therapy will be scheduled on the same day and time each week for consistency.

If you have questions or concerns as to how to explain your visit to TheraPlay Spot to your child please call for individualized support depending on the age and concerns of your child. TheraPlay Spot is a warm and friendly environment and you can reassure your child that they will have a fun and positive experience. The therapist will leave it up to you if you would like to observe your child during the evaluation. School Excused Absence slips can be provided upon request.
